Mental Health Targeted Case Management (MHTCM)

Targeted case management services are services to clients within targeted groups. The target population that may receive Mental Health Targeted Case Management (MHTCM) as part of the Texas Medicaid Program are clients, regardless of age, with a single diagnosis of chronic mental illness or a combination of chronic mental illnesses as defined in the latest edition of the American Psychiatric Association’s (APA), Diagnostic and Statistical Manual of Mental Health Disorders (DSM) and who have been determined via a uniform assessment process to be in need of MHTCM services. MHTCM services are furnished to assist clients in gaining access to needed medical, social/behavioral, educational, and other services and supports.

  • 1. Intensive case management services are predominantly community-based case management activities provided to the client or to the LAR on behalf of the client (who may or may not be present), to assist a client and care giver or LAR in obtaining and coordinating access to necessary care and services appropriate to the client’s needs.
  • 2. Routine case management services are primarily office-based case management activities that assist a client, caregiver, or LAR in obtaining and coordinating access to necessary care and services appropriate to the client’s needs.

Mental Health Rehabilitative Services

Mental health rehabilitative services are defined as providing assistance in maintaining or improving functioning and may be considered rehabilitative when necessary to help a client achieve a rehabilitation goal as defined in the treatment plan. Mental health rehabilitative services may be provided to a client with a serious mental illness as defined in the latest edition of the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM).
Mental health rehabilitative services may include:
• Medication management
• Psychosocial rehabilitative services
• Skills training and development
• Crisis intervention services

Skills Building and Development

Skills building and development is training provided to an eligible client, the LAR, or primary caregiver on behalf of an eligible client. The training addresses:
Serious mental illness and symptoms related problems that interfere with the client’s functioning and living, working, and learning environment

Provides opportunities for the client to acquire and improve skills needed to function as appropriatelyand independently as possible in the community.

Treatment Planning

Treatment planning is a process in which the therapist tailors, to the greatest extent possible, the
application of available treatment resources to each client’s individual goals and needs.

Community Based Intervention

This service is a family-focused, culturally competent and community based model providing time limited, intensive services to help improve the lives of children with complex needs and reduce the need of more restrictive settings including and especially out of home placements.

 

Counseling & Community Support

This is a face-to-face service that assists the client in restoring or strengthening skills needed to promote independence and stability. Clinical models and Evidenced Based Practices are used to increase consumers ability to modify negative behaviors, reduce symptoms of mental illness, and improve their overall quality of life.

 

Diagnostic Assessments

A Diagnostic Assessment is an intensive clinical and functional face-to-face evaluation of a client’s mental health, developmental disability, or substance abuse condition. This results in the issuance of a Diagnostic/Assessment report with a recommendation regarding whether the recipient meets entrance criteria, and includes an order for Enhanced Benefit services that provides the basis for the development of an initial person centered Treatment Plan. The assessment will be reviewed and signed by two Licensed Professionals.

 

Medication Management

Medical interventions that include assessments, prescriptions, supervision or administration of mental-health related medications, monitoring and interpreting results of laboratory diagnostic procedures that are related to psychotropic medications. Services include monitoring the side effects and interactions of medication and the adverse reactions which a consumer may experience and providing education and direction for symptom and medication self-management.

 

Trauma Focused

TF-CBT is an evidenced-based program with the goal of symptom reduction for youth who have been diagnosed with Post Traumatic Stress Disorder (PTSD) and/or have behavioral difficulties stemming from sexual abuse or other traumas. At least 80 percent of children treated in the program show marked improvement with 12 to 16 sessions demonstrated by:

-Reduction of negative emotions and behavioral responses to the traumatic events -Corrected maladaptive unuseful beliefs and attributions related to the abusive experience
-Support the non-offending parent(s)/caregiver while helping them cope effectively with their own emotional stress
-Help to build parenting skills that will help them respond optimally to and offer support to their children
Ages 5 – 18 and up to 21 of still living with parents

Trauma, Anxiety, Grief and loss, Depression, Anger management, Domestic violence, Child abuse, Sexual abuse.

All of our therapists are Licensed Independent Social Workers (LISWs), Licensed Professional Counselors (LPCs) or Licensed Graduates (LGs). Therapy may be provided at different sites including our offices, or the client’s school, home or other community settings, according to individual needs.

Counseling is provided within the context of the goals identified in the consumer’s treatment plans with the goal to help the client achieve and maintain stability and improve their physical, mental and emotional health.

TIP (Transition to Independence Process)

A collaboration process with the client where they decide the course of treatment. They are provided with developmentally appropriate, appealing services and supports. The family are important key players are instrumental participants who help the process in moving the client towards self-sufficiency and successful achievement of their goals.

We specialize in behavioral disorders, including, but not limited to:
